Insurance:

I am in-network with Premera Blue Cross Blue Shield and Lifewise.

I am out-of-network with all other insurance plans. Depending on your insurance, your coverage may include out-of-network benefits. Please contact your insurance company to gain more information about your plan. If you do have out-of-network benefits, you will be charged my cash rate at the time of the appointment, then provided a monthly “superbill” (a statement that includes diagnosis and basic session information required for billing) that can be submitted to your insurance.

Fees:

All fees are due at the time of service. Accepted forms of payment include debit, credit, Health Savings Account (HSA), or Flex Spending Account (FSA). Cards will be added to my secure client portal and will be charged the day of your session.

I do not provide occupational therapy, speech therapy, feeding/eating therapy, or ABA.
